Click the button under services and select your package from QDM’s lab shop and pay for services.

Once your lab test kits arrives, complete the test kit per instructions and ship (pre-paid packaging included).

After test analysis has been finalized, you will receive your test results and written recommendations from your health coach.

Advanced lab testing ordered by a doctor.

Written personalized in-depth recommendations by your health coach based on your individual laboratory results.

Medical Diagnoses - we do not diagnose medical illness or disease

Supplements - your health coach may recommend supplements and because test results are highly personalized, the cost of recommended supplements is separate from the test cost.

Blood Draws - if you choose the Nutrient Profile test, you will need to have a blood draw. A list of locations offering this service can be found in our lab store. The cost of the blood draw is separate and will need to be paid directly to the laboratory performing services.

Phone or video coaching. Phone and video coaching maybe available to you for a fee based on your wellness coach’s discretion.
